Practice Policy                                    New Patient Registration Patient Medical History

Malcomson Dentistry is a ‘Fee for Service’ dental practice.  We do not accept insurance. Payment for dental services will be due in full, at the completion of your appointment.  When lab costs are involved, and the recommended treatment plan is accepted, patients provide a 50% deposit, with the remainder due at the completion of treatment.  A payment plan for services rendered may be possible, when first discussed with the Practice Manager and agreed upon by Dr. Malcomson.

What is the advantage of a ‘Fee for Service’ practice?

Often insurance companies interfere with the Dentist / Patient relationship.  This allows insurance companies to dictate timing of dental procedure(s), and at times can compromise a patient’s oral health.  If a patient has dental insurance, Malcomson Dentistry staff will provide all necessary documents for the patient to submit and receive direct reimbursement from their insurance provider. CLICK HERE for ADA form.  Treatment plans are provided for pending dental services, so that our patients may contact their insurance provider to determine ‘out of pocket’ expenses. Malcomson Dentistry staff assists Patients in planning and maximizing year-end dental benefits and/or a health savings account.


 

Your 1st visit with Dr. Denise Malcomson…

New patients are required to complete the following forms: Patient Registration, Medical History, and Radiograph & Records Release Request (Patient Registration Packet).  Patients will also be provided a HIPAA consent form at their first appointment.  *Plan to arrive ten (10) minutes ahead of your scheduled appointment time in order to complete registration.

  • Radiographs (x-rays) allow Dr. Malcomson to have a clear picture of your teeth and bone level surrounding your teeth.  Bitewing radiographs (BWX) are recommended yearly to evaluate possible tooth decay.  If a patient can provide recent radiographs from their previous Dentist, duplicates may not be required.  If you have BWX, Full Mouth Series (FMX) and/or a Panoramic radiograph, they can be forwarded to MalcomsonDentistry@gmail.com.

  • Dr. Malcomson performs a full dental examination with periodontal probing, review of patient’s dental history and most recent radiographs.  Patients will always receive a Treatment Plan specific to their dental needs and wants.

  • Don’t forget to schedule your next appointment… Semi-annual hygiene appointments are scheduled at six-month intervals.  Malcomson Dentistry does not accept insurance, however, most dental insurance providers allow up to two hygiene appointments per calendar year.  Please contact your insurance provider for your specific policy coverage.  Depending on your oral health, age and lifestyle, Dr. Malcomson may recommend more frequent visits.  *This is only a recommendation.

Malcomson Dentistry patients receive a comprehensive Treatment Plan to financially prepare…

Our patients will be provided a comprehensive plan in writing.  Your dental treatment plan outlines exactly what dental services Dr. Malcomson recommends.  Patients can then budget accordingly, contact their insurance provider to determine ‘out of pocket’ expenses, and ultimately decide on the best course of action and time frame.  Patients can maximize any unused health benefits that they may have at year end on their Health Savings Account (HSA) or Flexible Spending Account (FSA).

Our Treatment Plans are detailed and specific to your dental needs and desires.  The path to a healthier mouth starts with your dental treatment plan, it is developed just for you, and based on your individual dental needs.  We’ll work together, to ensure your mouth is healthy and you’re smiling bright for years to come!

Your Treatment Plan will outline the specific dental services (ADA codes), time frame, and associated fees for said procedure.  It is a comprehensive, ‘big-picture’ approach to your oral care, designed to prevent small issues from getting bigger and more expensive.
